FocusHealth Medical Group participates in a variety of health plans. Before scheduling an appointment, contact your health insurance provider to make sure your doctor participates in its network and to understand your coverage benefits and costs.

Be sure to bring your insurance or Medicare identification card to your doctor’s appointment. Your insurance may require prior authorization before seeing a doctor and you may be called for insurance or authorization information before your visit.

Check if your insurance requires an office copayment — the fixed amount you pay for a medical service. If so, you will be asked to make your copayment in full.
